The ASN, PN, Dental Hygiene, and CNA programs all have clearance requirements IN ADDITION to the Registrar's clearance requirements. There is some overlap, which can make it more confusing, but keep in mind that you have to upload all of the Registrar's documents to Med+Proctor, and all of your Allied Health Program's documents to Clinical Wallet. Even if they seem to be the same requirement, they have to be uploaded to both sites. Where they have similar requirements, follow the more stringent requirements

Shortly after you are notified of your acceptance into your program, you should receive an email from Clinical Wallet with instructions entitled "Quick Start for Students." This will walk you through setting up an account, how important it is to use your UH email address, how to select the program and class you belong to, etc. You will also need to select and pay for the Pro plan, which is $60 per year unless you have been given a code to enter. 

Are you having trouble getting documents into PDF format? The best app we have found for this is Genius Scan. You just take a picture of your document with the app, and it magically cleans it, flattens it, and makes it readable. You can easily upload it straight from the app to iCloud, or Drive, etc. so you have access to these precious documents forever.

Why does your Allied Health Program have additional clearance requirements? You will be providing healthcare to patients at the dental facility on campus, at different clinical sites throughout Maui, or even on other islands. All of our clinical site partners have their own requirements for providing these opportunities for our students, and it is our responsibility to ensure our students meet all of them. The vaccines, physical exams, CPR certifications, etc. help the clinical sites know that you will be safe candidates to provide care to their patients.

ASSOCIATE OF SCIENCE IN NURSING PROGRAM 

- Allied Health Student History & Physical Exam Form & Technical Standards Form. You will need to schedule a program clearance physical exam at the UHMC Health Center (easier), or with your PCP. Please print and take these forms with you! When complete, upload them to Clinical Wallet. 

- Allied Health Additional Standards Form. You can fill this form out yourself, then upload it to Clinical Wallet.

- Documentation of 2 MMR vaccinations. Upload proof of your vaccinations to Clinical Wallet. 

- Documentation of 2 Varicella vaccinations. Upload proof of your vaccinations to Clinical Wallet. 

- Tdap vaccination within the last 10 years (note - this is different from the Registrar which asks for a Tdap after the age of 10). Upload proof of your vaccinations to Clinical Wallet. 

- Documentation of 3 Hepatitis B vaccinations or a positive titer showing immunity. Note: please just upload the documentation of your vaccinations to Clinical Wallet if you have it. Some clinical sites ask that you repeat the vaccination series if you have a negative titer.

- COVID-19 vaccination updated by time of start of clinicals - AKA whatever new version comes out the Fall your program starts. 

- Flu vaccination updated by time of start of clinicals. AKA the vaccine that is released the Fall your program starts. 

- TB Healthcare clearance per HI State Guidelines - get this modified HI TB Doc F form completed by a healthcare provider showing:

  • 2 step TB test or
  • History of 2-step TB test with annual screening every year after that or
  • Negative quantiFERON gold blood test or 
  • If a history of positive skin test, a chest x-ray within the last year, or chest x-ray with annual screening every year since

- Criminal background check. Go to the State of Hawaii site here, create an account if you do not have one, search for yourself, order your record, and download the PDF to upload to Clinical Wallet.

- Valid health insurance card. Many of our students report that they work too many hours to qualify for Quest - and that won't change until they start classes. Please let us know if that is your situation. Keep in mind, if you are 26 or under, you can still be covered by your parents' insurance. Also Maui has a wonderful service called IMUA Healthcare Navigator where someone will help you figure out how to get covered!

- Copy of your State-issued ID (driver's license, etc.). Just take a picture and upload it to Clinical Wallet. 

UHMC Health Center Forms:

- UHMC Release of Records Form for Clearances. This form allows us to send your forms and vaccine records if requested to your clinical placement sites. 

- UHMC Registration & Fee Schedule 2024. This form gives the Health Center your contact information, and gives you information about our extremely low fees. 

Program Required Forms:

- MHS MRI Safety Screen Form. Maui Memorial Medical Center and Kula Hospital ask that all our students complete this form that screens for whether you can safely be around an MRI system. It uses very strong magnets, so staff needs to know if you have metal in your body. Please complete the form, and upload it to Clinical Wallet. 

- OPH Notice of Confidentiality Practices. This form is for Hale Makua locations, and OPH asks that all of our incoming students complete it. Please read, sign, and upload to Clinical Wallet.

- OSHA Certification - your program will arrange for your OSHA compliance for Healthcare Professionals training, likely to be completed in August. After this, you will need to upload documentation of completion to Clinical Wallet.

Question: Where would I find my vaccination records?

Answer: MMR, Varicella, Tdap, and for most of you, Hep B vaccinations would have been required to register for High School or for wherever you completed your prerequisite courses. So you probably had them done, and at some point, you or your parents got these records from your pediatrician or PCP. Are they in a file at home? Are they attached in an email from when you requested them to register? Does your care provider have the records? Does your last school still have them? If you have Kaiser, have you checked KP.org? If you have another provider, do they have an online portal you can check?

Question: I have looked everywhere and I cannot find any records of past vaccinations, but I know I had them. What do I do? 

Answer: You will have to repeat them, which is safe to do. If you have already received the vaccinations, you will likely have no reaction to them. 

Question: Where can I get my vaccinations, TB tests, and physical exam done? 

Answer: The on-campus UHMC Health Center 808-984-3593 will be the most convenient place to receive all of these services. We offer low-cost exams with little-to-no wait, and charge only administration fees for the vaccines. For instance, while a retail cost for the Varicella vaccine is currently $174, we only charge a $15 administration fee per vaccine. We also provide 2-step TB tests, order quantiFERON gold blood tests, and refer for chest x-rays when indicated. Your PCP may offer some of these services, but usually not all, and will be much less familiar with the complicated details of the clearance requirements. The County's Wailuku Health Center offers free TB testing on Tuesdays and Thursdays, but does not offer exams or vaccinations. 

Question: Why doesn't the Allied Health Department accept titers (blood tests) for proof of immunity for anything other than Hepatitis B? 

Answer: Since 2020, the University of Hawaii Registrar stopped being able to accept titers in place of proof of MMR and Varicella vaccination records. Since we know our students will need these vaccines for the Registrar, we adjusted our requirements to be consistent. However, since Hepatitis B immunity is not a requirement for UH registration, and all of our clinical sites accept titers for this, we can accept it in this case.

Question: I uploaded my MMR, varicella, and Tdap vaccination to Clinical Wallet already. Since you are all the same organization, why won't the Registrar just see those records and let me register for classes? 

Answer: You do still have to upload those vaccination records to the Registrar's database at Med+Proctor. The Registrar cannot access the Clinical Wallet database, and we cannot access the Med+Proctor database. Additionally, they have different features. Clinical Wallet can generate reports that can be sent to our clinical partners, while Med+Proctor integrates with registration software. 

Question: I did a TB Screen for the Registrar and it was accepted. Why is Clinical Wallet rejecting it?

Answer: TB Healthcare Clearance is required for any Allied Health Program, and it is much more extensive than a TB Screen. Instead of a questionnaire, it typically involves a two-step TB skin test. You can call us if you have questions. Please complete those requirements and upload the completed HI Doc F to Clinical Wallet. 

Question: The Registrar says I am exempt from the MMR vaccination requirement because I was born before 1957, or exempt Varicella vaccination because I was from born in the United States prior to 1980. Why is Clinical Wallet still demanding these vaccines? 

Answer: These exemptions for the Registrar are not exemptions accepted by our clinical partners, so we still need to require that our students be vaccinated. 

Question: My Tdap was given after the age of 10 like the Registrar said it should be. Why is Clinical Wallet rejecting it?

Answer: If you look closely, our requirement is that the Tdap be given within the past 10 years. This is because the Tdap is considered due for re-vaccination after 10 years.

Question: My PCP gave me a Td vaccine instead of a Tdap. Isn't that good enough? Will it be safe to get a Tdap if I just got a Td? 

Answer: Our clinical partners require the Tdap because it protects you and the patients you will be caring for against pertussis. This respiratory illness can be fatal to the very young, the very old, and to the immunologically compromised. It is safe to get a Tdap regardless of the interval since the last Td vaccine according to the CDC

Question: Are there any exceptions to the above requirements? 

Answer: Getting an exception to a clearance vaccination when you are going into a medical field is more difficult than under other circumstances, but depending on your circumstances, or your clinical site there may be exceptions or an option to sign a declination form. However, please consider getting all of the required clearance vaccinations as they are for your protection and for the protection of your patients.
